On December 2, 2025, the Live Healthy Spartanburg team celebrated the completion of the Community Health Improvement Plan (CHIP) Report alongside coalition partners, Community Advisory Board members, and resident ambassadors. Hosted at The Spartanburg County Foundation Center for Philanthropy, the event recognized the hard work and 60+ hours invested into developing the 2026–2028 Community Health Improvement Plan.

This report serves as a strategic roadmap outlining our county priority areas, indicators, strategies, and performance measures.

What is a Community Health Improvement Plan?

A Community Health Improvement Plan (CHIP) is an action-focused document that identifies priority health issues based on data from the Community Health Needs Assessment. CHIPs are created through a collaborative, community-wide process that brings together partners, residents, and organizations to design, support, and implement solutions to improve long-term health outcomes.

For Spartanburg County, the four priority areas include:

  • Access to Care
  • Behavioral Health
  • Healthy Eating & Active Living
  • Resilient Children & Families

By aligning goals, coordinating resources, and leveraging local leadership, Live Healthy Spartanburg convenes partners to drive sustainable change and address the root causes of health and wellness challenges in our community.

What’s New in Our 2026–2028 Community Health Improvement Plan (CHIP)?

Live Healthy Spartanburg is excited to share that our 2026–2028 CHIP introduces a major shift in how we measure progress and drive impact. For the first time, our coalition has adopted the Results-Based Accountability (RBA) framework. This approach adds a clear measure of accountability and helps us answer three essential questions:

  1. How much did we do?
  2. How well did we do it?
  3. Is anyone better off?

By focusing on these questions, RBA ensures we’re not just tracking activities but also outcomes that truly matter for community health. This framework aligns perfectly with the principles of collective impact, allowing partners to work toward shared goals with transparent metrics and a common language for success. The benefit? Greater clarity, stronger collaboration, and measurable improvements for Spartanburg County.
